The application form for rest space in each area and building

This is the application form for using the women's rest rooms on the 2nd floor of Building A and B of the Information Science Building, the 1st floor of the Bioscience Building, and the 1st floor of Building F of the Materials Science Building.

When other users check the usage status, the app will display the QrioLock account name of each user. When other users check the usage status, the app will display the name of each user's QrioLock account. In order to anonymize the name that appears in this history, please set an arbitrary name that does not identify the individual, such as initials.

※For those who wish to use the restroom in Bio Building.
Due to the limited number of electronic keys that can be made on the app, there will be those who will be given a card key instead. We will assign an anonymous QrioLock account name to the card name, but you will not be able to use the application when you use the card key, so you will not be able to view your history. Please be aware of this in advance.

※For those who register on the app.
When you create a QrioLock account, please register the name that you reported here. This will allow the Office for Gender Equality to match the name of the registrant with the name of the QrioLock account.

★Please register with the same e-mail address as the one you will provide when you apply to use the restroom. If you tap "Use the duplicate key" in the body of the e-mail, you will be able to unlock and lock the electronic lock and use the rest room.（The duplicate key information is only valid for 24 hours after issuance.) In the case of a card key, we will contact you by e-mail with instructions on how to unlock and lock the door.
